Who We Are

Full Precision Health PLLC ("FPH," "we," "our," "us") is a telehealth registered-dietitian practice. We provide medical nutrition therapy, laboratory ordering, and coordinated medical oversight to patients in the states where we are licensed. We are a covered entity under the Health Insurance Portability and Accountability Act (HIPAA).

Information We Collect

Identifiers and contact information (name, date of birth, address, email, phone).

Insurance information (carrier, member ID, plan details) provided for benefit verification and billing.

Protected health information (PHI) you provide during intake, visits, or messaging, including medical history, medications, symptoms, goals, and laboratory results.

Payment information, processed through PCI-compliant payment processors. We do not store full card numbers on our servers.

Website usage data (pages viewed, device type, approximate location) collected through first-party analytics only if you accept optional cookies. See our Cookie Policy.

How We Use Your Information

To provide clinical care, including nutrition counseling, protocol design, laboratory review, and coordination with prescribing clinicians.

To verify insurance eligibility and to bill your health plan for covered services.

To communicate about appointments, care instructions, and follow-up.

To comply with legal, regulatory, and accreditation obligations, including HIPAA.

To improve our services and website, using aggregated and de-identified data.

How We Share Your Information

Treatment partners: physicians, pharmacies, laboratories, and other clinicians involved in your care.

Business associates: technology vendors (EHR, scheduling, messaging, payment, analytics) who sign a HIPAA-compliant Business Associate Agreement before handling PHI on our behalf.

Health plans and payers: for eligibility verification, claims submission, and benefit coordination.

As required by law: public health reporting, judicial orders, or to prevent serious harm.

We do not sell your personal information. We do not use your PHI for marketing without your written authorization.

Your Rights

You have the right to access, amend, and receive a copy of your medical record.

You have the right to request restrictions on how your PHI is used or disclosed.

You have the right to receive an accounting of disclosures made outside of treatment, payment, or operations.

You have the right to file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, without retaliation.

Data Security

We encrypt PHI in transit and at rest, limit access on a need-to-know basis, and maintain administrative, physical, and technical safeguards required by HIPAA. In the event of a breach affecting your PHI, we will notify you in accordance with applicable law.

Vendors

Vercel hosts this website. Optional first-party analytics (window.va) load only after you accept cookies, and never when your browser sends Global Privacy Control or Do Not Track.

Typefaces will be self-hosted. We do not load Google Fonts, advertising pixels, or a third-party scheduling iframe in the browser.

Cookies, Analytics, and Global Privacy Control

Our public website uses necessary storage to function. Optional first-party analytics load only after you accept. If your browser sends Global Privacy Control (GPC) or Do Not Track, analytics stay off even if a previous visit accepted them. Fonts will be self-hosted. We do not run advertising pixels on this site.

Children

Our services are directed to adults. We do not knowingly collect personal information from children under 13 through this website. If you believe we have collected information from a child in error, contact us so we can delete it.
