Acceptance of These Terms
By accessing or using fullprecisionhealth.com (the "Site") or any services offered by Full Precision Health PLLC ("FPH," "we," "our," "us"), you agree to these Terms of Use. If you do not agree, do not use the Site or our services.
Nature of Our Services
FPH provides telehealth registered-dietitian care, medical nutrition therapy, and coordinated medical oversight in the states where we are licensed. Access to specific medications, laboratory tests, and clinical services depends on licensure, clinical appropriateness, and insurance coverage.
The Site and its content are informational and do not constitute medical advice. A clinical relationship is only established after you complete intake, are accepted as a patient, and meet with a licensed clinician.
Our services are not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Eligibility
You must be 18 or older and located in a state where we are licensed to use our clinical services. Certain services have additional clinical criteria. We reserve the right to decline or discontinue care in our clinical judgment.
Insurance and Payment
We verify insurance benefits as a courtesy before your first visit. Verification is not a guarantee of payment by your plan. Final coverage is determined by your insurer based on your plan, medical necessity, and claim adjudication. You remain responsible for any amounts not paid by your insurer, including deductibles, copays, and coinsurance.
Self-pay prices and add-on service fees are published on the Site and may be updated. Laboratory and medication costs are separate and may be billed by the laboratory, pharmacy, or your insurer.
Your Account and Communications
You agree to provide accurate and complete information and to keep your contact details current. You are responsible for activity in your account and for securing your login credentials.
By providing your phone number and email, you consent to receive appointment reminders, clinical messages, and account notifications. You can opt out of marketing communications at any time; you cannot opt out of communications required for your care while you are an active patient.
Intellectual Property
The Site, its design, copy, imagery, logos, and software are owned by FPH or our licensors and are protected by copyright, trademark, and other laws. You may not copy, modify, distribute, or create derivative works without our prior written consent, except for limited personal, non-commercial use.
Acceptable Use
You may not use the Site to: attempt to gain unauthorized access to any systems; interfere with Site operation; upload malicious code; scrape content for commercial use; misrepresent your identity; or violate applicable law.
Third-Party Services
The Site may link to or integrate with third-party services (e.g., scheduling, payment, laboratory portals). Those services are governed by their own terms and privacy policies. We are not responsible for their content or practices.
Disclaimers
The Site is provided "as is" without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of harmful components.
Clinical outcomes vary. Testimonials and results referenced on the Site represent individual experiences and are not a guarantee of future results.
Limitation of Liability
To the fullest extent permitted by law, FPH and its owners, employees, and contractors are not liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the Site, except where prohibited. Our total liability for any claim arising from the Site is limited to the greater of one hundred dollars ($100) or the amount you paid us for the service giving rise to the claim in the preceding twelve months.
Nothing in these Terms limits liability that cannot be limited under applicable law, including for gross negligence, willful misconduct, or personal injury caused by our negligence.
Governing Law
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-laws rules. Any dispute arising from these Terms or the Site will be brought in the state or federal courts located in Miami-Dade County, Florida, unless applicable law requires otherwise.
Changes to These Terms
We may update these Terms as our services or the law change. The "Last updated" date at the top reflects the most recent revision. Continued use of the Site after a revision constitutes acceptance of the updated Terms.
